Self-Enrollment for MyCovenantHealth

Covenant Health of East Tennessee patients can self-enroll for MyCovenantHealth, where you can securely view your medical information and connect with your healthcare team. You must be at least 18 years old to enroll in MyCovenantHealth.

You may self-enroll using your email address that you provided at the time of your visit, or personal identifiers, which include your Medical Record Number and the last four digits of your Social Security number.

Self-enrollment is not currently available to caregivers or guardians.

To self-enroll, you will need the following information, and it must match what you provided to us during registration at our facility:  

1. First Name
2. Last Name
3. Date of Birth

Next choose how you want to verify your account:

2. Medical Record Number + Last four digits of your Social Security number
